Evidence Based Medicine- An Inquiry into Effectiveness and Efficiency
Clinical practice seems a bombardment of conventional and unconventional approaches towards that of the maladies plaguing bodily form and function. The very culmination of the medical establishment as a therapeutic and social force. With an armamentarium of physicians on a single dimension of the sphere and the myriad of accessible literature on another, it stands as a provocative establishment of prestige and clinical intervention. Medicine, as a force conquering those physiological fronts is at the threshold of an unprecedented internal conflict. One that foresees the plaguing of its institutions with dogma. The suppression of those innovating and inquisitive clinicians driven to make a profound impression. The advent of evidence based medicine as a clinical and medical utilisation has provoked reaction from both ends of the establishment. An institution from which its modest beginnings one can draw an evident line of effectiveness and efficiency. A striving to draw the best clinical evidence via that of the scientific method. An uncompromising state of clinical and medical advance; marked by scientific basis to modern medical practice. A fabrication of the controlled clinical trial, the very gold-standard of clinical based knowledge and the prioritisation of the individual values and expectations. It is truly a miraculous evolutionary tale. How a concept doused liberally with altruism and cutting edge clinical execution. An expression of the underlying effectiveness and efficiency bound by a passion to restrict harm and reinforce that of the good. Undeniably, an entire host of those physicians and clinicians fail to conquer the essence of such an invaluable reality. Capitalising on the exclusive tenets of the evidence based medicine and posing obliviously to the observation and execution that it proposes. Evidence based medicine as an observation and as an execution is manifestly professed with the genesis of the randomised control trial. With the advent of maladies, disease and disorder that teased the likes of those leading physicians, where did the evidence lie as a basis for their practise? And did they reinforce harm and ignore pleas for that of the good? One of the most preeminent tales of the evidence based genesis arose with that of Professor Archie Cochrane. Pioneer of the controlled trial and Edison of the modern evidence based practice; his name remains synonymous with the scientific method in and around the establishment. While championing for evidence, Cochrane acknowledged the cruciality of the randomised control trial. Placing arbitrary individuals in a pair of arbitrary ‘controlled’ and ‘experimental’ categories, he rewrote the dogma and converged on a front no innovative physician had ventured. In more contemporary times, Cochrane’s paradigm stands as memorial as few regard its role and many regard is misinterpretation. For as long as we are instigating medical and clinical advance, we will be approached with scepticism and mistrust. We will be interrogated and asked of any ‘evidence to substantiate our toil’ and our ever move placed under the stethoscope. It is this mass misinterpretation of evidence based medicine that breeds the depletion of the values it originally aimed to substantiate. Depletion and a diminishing of the fabric of the establishment and those innovating physicians. "I knew that there was no real evidence that anything we had to offer had any effect on tuberculosis, and I was afraid that I shortened the lives of some of my friends by unnecessary intervention”, proclaimed Cochrane as a frontier was incurring an impending, yet profound transfiguring….

Desire- A Facet of Being
Desire remains the definitive stigma. Bound by the entity it entails and the circles of individuals that set out to anatomise its form and function. A critically accesible and attainable facet of being, an undermined faculty of the psyche. The culmination of thought, feeling and emotion on a personal and subjective front. Perpetuating the cravings, inclinations and yearnings that fulfil the self to either a compromising or uncompromising state of being. The summit of human psychology, a faculty of inquiry that continues over a consistant basis to tease the likes of our most preeminent thinkers. Desire is ever worthy of a combination of scrutiny, acclaim and examination.To anatomise and fully dissect the technicalities and eccentricities of desire, one must conquer from every approchable angle the focal point at which one is motivated and driven towards those yearnings and inclinations. A focal point of our individual and collective predisposition in close conjunction with a psychological and a neurologic basis. The very focal point remains the pinnacle of any desire to conquer desire. Desire is a manifestation, a facet and state of being. One driven by neither the electrochemical processes of neurotransmitters but by an ever immanent and transcendent force. The current state of desire throughout many of our academic and intellectual circles is worthy of criticism. Its profound undermining by those theological and religious clerics and the flawed interpretations by those claiming to attain a higher order has significantly and rapidly ressected from its once, premised position in the literature and the annals of thought. The very focal point is a direct representation of the psyche, the self and its state of being. Symbolising the culmination of moral advance or failure, yearning and inclination and a passion of wisdom. It is the very inquiring and inquisitive mind that will recognise a reality of truth and sheer essence through this focal point. A point of the highest essence and concentration that when one sets out to conquer it or document its abstraction he is overwhelmed and mesmerised. Many societies continue to perpetuate this stigma towards that of desire. Labelling it as a disorder, an incurable malady of the heart. The inquisitive minded recognises 'a reality substantial to the world and things and minds. A psyche, a self that when exposed to the intricate proportion of stimuli. The stigma remains reinforced by the over-emphasis of the evil and satanic entities pertaining to that of the self and its inclinations and yearnings. Whereas there remains an incredibly diverse and superfluous expression of the pure and unadulterated self that formulates the focal point. Desire comes much from the unprecedented effects particular stimuli have on the self, from a psychological and mystical front. A catalyst which may reinforce and provoke negatively oriented desire will drive the psyche to such a state of moral failure. But an inhibition that dissolves those negatively oriented drives and reinforce a positive and desirable psychology and a state of being. What must be critically acknowledged is the prospect that the self is an invaluable facet of being and that the numerous attempts by flawed and often compromised approach seek to diminish the sheer essence of desire and its constituents. And as the thinker, the logician seeks to ponder that higher and transcendent reality he converges on those fronts never before ventured.

Pathology of a Philosophy- The Cessation of Thought
Much time has passed since I have addressed my readers from a personal standpoint. It has occurred to me after much meticulous contemplation that the very germ of a revolutionary feat deeply rooted within that of thought is pervious to a gradual decline and a swift demise. One where the facets of intellect and the tenets of wisdom lose their intrinsic worth; succumb to a plague of narrow-mindedness and a collective ignorance. One where thought itself is viewed as a mere localised faculty of the mystical mind and the physiological brain. A transcendent and immanent philosophical doom, the cessation of a renaissance. The renaissance is often attributed to the conventional flourishing artistic and scientific ambience pertaining to that of the past. But a more innate and intrinsic connotation is apparent. The renaissance as a ‘rebirth’ seems the definitive expression of the philosophical and thought based progress made on a personal, collective front. But just as the very germ of a concept was being fertilised, its demise was being conspired. The very thought nexus, the mother of all philosophical expressions and conundrums is at last approachable from that of a pathological platform. But for one to perceive the sheer enormity of the nexus, one must conceive its anatomy, its meticulous physiology ultimately leading to the realisation of its pathologic short-comings. The death of the philosophical renaissance, the rebirth of the nexus is coherent and ever logical. For when a society, a circle of individuals and intellectuals embark on that of a breakthrough, the responsibility to maintain it and preserve its essence becomes an undeniable obligation. The nexus seemed impregnable and impervious to destruction when we discussed its proportions regarding that of ‘Tablet and Pen’, how those literary landscapes pave the way to those indefinite, yet infinite series of intimate interconnections. Some may propose the elimination of our literary mastery as the junction between the nexus and its destruction. That proposal is ever copious with flaws for the nexus is a direct and uncompromising representation of the whole of thought, wisdom and intellect. A pathologist may inform you the disorder may have implications targeting that of the anatomical and physiological fronts of an organism. Nonetheless, the philosopher will indicate the lack of intellectual spirit, the fiery passion to institutionalise a revolution represents as onset of the destruction of the nexus. Miraculously, the thought nexus remains a transcendent, invisible and unseen reality. Yet it fulfils the intellectual criteria of accessible, attainable and ‘portrayable’. Yet this leads to yet another conundrum, has the nexus been slaughtered or has our perception and conception of its presence been profoundly diminished to the core? Philosophy has been slow to employ and adopt the model of the nexus. But our independent and literary account of the renaissance associated with that of its inception and the probable demise of our conception and perception of it has proved thorough and ever-encompassing. A striking impression upon the specialties, disciplines and vocations that claim to possess an insight or an envisioning into that of thought. For we remain preeminent, for when our conception and perception run low, we will possess that differential diagnosis mending the nexus, interconnection by interconnection…

Skin- A Personal Physiology
The very culmination of the physiological fronts. A mere three layered entity, posing as the sole basis for a cultural and social icon. The marriage of a biological manifestation with the twin realities of beauty and elegance. While bound by an essence, overlooked and disregarded by that of its possessors, much has arisen in the realm of form and function. Allowing much room for an intrinsic approach rather than the dogmatic conventions of the literature. Ordaining the very attainability and accessibility that comes with its ubiquitous and abundant being. Skin remains an ever perennial physiology. One of personal proportions, as the largest of the anatomical giants makes a profound impression upon the physiological processes that sustain the very psyche and harbour its nucleus. Skin is ever copious, a representation of the biological possibilites that arise simply from a three layered entity. The dexterity to register sensation, retain moisture; the capacity to protect and regulate. Such profound functions pertain to the skins indispensability, the junction between the inner and the outer, a shroud of physiological significance and intrinsic value. For when one gazes analytically at the myriad of tissue that constitute to its form, the very prospect of the function instantly boils deep within the electro-chemical processes of the mind. Giving rise to a sensuous perception of the psyche, as a tangible and accesible reality. The psyche remains the true self, the being; the skin allows the junction between that psyche and the organism that continues to harbor it. A form of recognition, a thought provoking stimuli, unsurpassed and almost spontaneous in nature. Moreover, the more personal dimension of the sphere reveals a more intimate, fidel and personal correlation. A relationship with that of anatomical form and physiological function that builds and consolidates the sensation of 'individuality'. An external reality, rather than an alien and foreign internal happening. For from the very extremity of the epidermis to deep within the ravines of the subcutaneous tissue, skin continues to emulate an unparalleled stance. A postion of preeminence, attributed predominantly to the protection, regulation and registering that falls under its roster of laborious toil. Anatomically, the significance of skin is its foundation of a realm entirely difficult to conceive and envision. One abundant in fatty deposits and neural connections. Profuse in layer upon layer upon layer. Ample with familiar pores that give rise to some of the coarsest to much of the finest hairs. There remains an evident line of malfunction therein. A myriad, a stream of disorder that threatens the physiological processes it consolidates and the anatomical structure behind its intricacy and elegance. The symbolic and definitive beauty, an entity we yearn to perfect, with a combination of the precise complexion and proportion. Skin remains an overlooked reality. An overlooking perpetuated by our wonder with the neuronal interconnections and the cardiological fronts, an impersonal appeal for the very bases of the body. So in turn it becomes paramount, of the highest importance and cruciality to place a substatial degree of personal appeal and regard to that of the skin as a physiological entity of proportions surpassing that of the dogmatic, impersonal approaches...

The Dystopia- Dissecting a Literary Species
Often restricted to the space of that of a novel or play. Its implications have held paradigm shifting proportions as it exceeds the realities of reality. Converging on a political and social arena never before ventured.The culmination of the political theorists and the literary men, in conjunction with a gripping and riveting theme. The perpetuation of ideology within and around a realm bound by a passion for absolute governance. With a lack of opposition and copious volume of citizens at disposal, it has produced impressions and stances throughout the literary species. The Dystopia, has profoundly transfigured the social and political fronts, in conjunction with the public perception of political and social rivalry. Allowing the envisioning of a state that holds absolute governance upon the public mentality and the liberties we often overlook. It is the dystopia which has truly extended the boundaries of modern literature and doused it with the horrific and diabolical themes of mind control and pervasive means of surveillance. A literary model bent on a probable ideal for the near future. An ideal which shall foresee the international community bound by a single state, a single totalitarian governance and the repressive stances of that governance. From the revolutionary words penned by Aldous Huxley in his 1931 dystopian novel, Brave New World, to the gripping tale portrayed by George Orwell in Nineteen Eighty Four, a literary species was invented and the public perception endured a paradigm-shift in its approach to the political and social powers they fall under. The literary species always been a myriad of essays and treatises, but with the inception of a dystopia, a more central and particular theme is being propagated. With the administering of drugs, the application of surveillance, widespread promiscuity and the ascent of mind control technique, the dystopian state reinforces its repression and unifies all under a single bureaucratic and oppressive banner. The sociological front has found refuge in the model of the dystopian state, often making analogies and comparisons between the conflicts of more contemporary societies and the dystopia. Meanwhile, the viability and feasibility of a dystopian model by many of the critically minded theorists. One may gather that with the rise of repressive bureaucracies, a trigger of the critically minded and resistant will be pulled, giving birth to the death of that state. Since the armamentarium of the dystopia holds mind control as a specialty, those opposing literary men and theorists take their stand. The dystopia as a literary species becomes approachable from a multitude of angles. Some would see fit in examining the political facet of the species, while others may seek interest in the economic and social implications. Constricting the proximity and authority of the dystopia as a literary species is provocative of a dull and unexaminable effect. The dystopia will become merely a theme, than a more grandiose and prominent political and social model. Thus, it becomes paramount to maintain and continually produce the literary species so central to the specialty of literature, for when a species such as the dystopia come to rise, the sole savior will be the very literature that brought it to such a state of prominence...
